Use

Therapeutic indications

- They come from the antibacterial activity and pharmacokinetic characteristics of this medicine. They take into account both the clinical studies it has led to and its place in the range of antibacterial products currently available.

- They are limited to infections due to germs recognized as sensitive, in particular to certain situations in which the bacterial species responsible for the infection can be multiple and / or resistant to the antibiotics currently available.

- This dose is indicated in infants younger than 30 months in:

. acute otitis media,

. lower respiratory tract infections,

. urinary tract infections.

- The official recommendations on the proper use of antibacterials must be taken into account.

Dosage and method of administration

Doses expressed as augmentin:

INFANTS

80 mg / kg / day in three doses. All three intakes are recommended to obtain sufficient serum concentrations at night.

Contraindications

CONTRAINDICATED:

- This medicine should NEVER BE USED in case of:

. allergy to beta-lactam antibiotics (penicillins, cephalosporins): consider the risk of cross allergy with antibiotics from the cephalosporin group,

. allergy to any of the components of the drug,

. a history of liver damage related to the amoxicillin / clavulanic acid combination,

. phenylketonuria, due to the presence of aspartame (E951).

- Due to the presence of maltodextrin (glucose), this medicine is contraindicated in cases of malabsorption of glucose and galactose.

RECOMMENDED

This medication is generally NOT RECOMMENDED when used in combination with methotrexate (see interactions section).

Warnings and precautions for use

PRECAUTIONS

- The appearance of any allergic manifestation requires the cessation of treatment and the establishment of an adequate treatment.

- Immuno-allergic reactions, including severe and sometimes fatal hypersensitivity reactions (anaphylaxis) have been exceptionally observed in patients treated with beta-lactam antibiotics.

- Therefore, its administration requires a previous examination.

- Before a history of typical allergy to these products, the contraindication is formal.

- Penicillin allergy intersects with cephalosporin allergy in 5-10% of cases. This leads to the banning of penicillins when the subject is allergic to cephalosporins.

- The appearance, at the start of treatment, of a generalized febrile erythema associated with pustules, should lead to suspicion of a generalized acute exanthematous pustulosis (see section on adverse effects: these cutaneous manifestations may be more frequent and / or more intense in patients with infectious mononucleosis or developing lymphoid leukemia); it requires stopping treatment and contraindicates any new administration of augmentin alone or in combination.

- Due to the presence of maltodextrin (glucose), this medicine is contraindicated in cases of malabsorption of glucose and galactose.

EMPLOYMENT PRECAUTIONS:

- As with all beta-lactam antibiotics, check your blood count regularly when administering high doses of amoxicillin.

- The administration of high doses of beta-lactams, in patients with renal failure or in patients with predisposing factors, such as a history of seizures, treated epilepsy or meningeal disorders, can exceptionally lead to seizures.

- In patients with renal failure, for creatinine clearance less than or equal to 30 ml / min, adjust the dose.

- The existence of low urine flow is a risk factor for the appearance of crystalluria.

- When high doses of augmentin are administered, sufficient fluid intake should be guaranteed to reduce the risk of crystalluria.

- The risk of the appearance of adverse liver effects (cholestasis) increases in case of treatment longer than 10 days and, in particular, more than 15 days.

- In case of prolonged treatment, it is recommended to control liver and kidney functions.

- The amoxicillin / clavulanic acid combination should be used with caution in case of liver damage.

Effects on biological parameters:

Direct Coombs test positivity was obtained during beta-lactam treatment. This could also occur in subjects treated with the amoxicillin / clavulanic acid combination. False positives from this test have also been observed related to the presence of clavulanic acid.

At very high concentrations, augmentin tends to:

. decrease blood glucose test results,

. interfering with the determination of the total serum protein level by color reaction,

. give a false positive color reaction in the glucosuria tests by the semiquantitative colorimetric method.

Interaction with other medicinal products and other forms of interaction

RECOMMENDED ASSOCIATION:

Methotrexate:

Increased effects and hematological toxicity of methotrexate by inhibition of renal tubular secretion by penicillins.

ASSOCIATION TO TAKE INTO ACCOUNT:

Alopurinol (and by extrapolation, other uricosynthesis inhibitors):

Increased risk of skin reactions.

SPECIFIC INR UNBALANCE PROBLEMS:

Many cases of increased oral anticoagulant activity have been reported in patients receiving antibiotics. The marked infectious or inflammatory context, the age and the general condition of the patient seem to be risk factors. In these circumstances, it seems difficult to draw a line between infectious pathology and its treatment in the appearance of INR imbalance. However, certain classes of antibiotics are more involved: these include fluoroquinolones, macrolides, cyclins, cotrimoxazole, and certain cephalosporins.

Undesirable effects

- Infections and infestations:

Common: mucosal cutaneous candidiasis.

- Blood and lymphatic system disorders:

. Eosinophilia has been reported.

. Rare: leukopenia (neutropenia), agranulocytosis, thrombocytopenia, and reversible hemolytic anemia.

- Immune system disorders:

Allergic manifestations including urticaria, angioedema, respiratory discomfort, very rare anaphylaxis (including anaphylactic shock), serum sickness, hypersensitivity vasculitis.

- Disorders of the nervous system:

. Uncommon: dizziness, headache.

. Very rare: seizures.

- Gastrointestinal disorders:

. Common: diarrhea, loose stools, nausea, vomiting.

. Uncommon: dyspepsia and abdominal pain.

. Very rare: pseudomembranous colitis and hemorrhagic colitis.

Superficial dental stains, generally reversible after brushing, have been reported very rarely in children with oral suspension forms.

- Hepatobiliary disorders:

. Uncommon: moderate and asymptomatic increase in ASAT, ALAT or alkaline phosphatase.

. Very rare: cholestatic or mixed jaundice and / or hepatitis.

These cases seem to occur more easily in elderly and male patients and during prolonged treatment (more than 10 days and in particular for treatments of more than 15 days).

These effects occur during treatment, but in some cases they may not appear until several weeks after stopping treatment.

Histopathological examination of the liver usually reveals central-lobar cholestasis.

The evolution is generally favorable, but it can last a few weeks. Very rare cases of fatal development have been observed.

- Skin and subcutaneous tissue disorders:

. Uncommon: skin rash, itching and hives.

. Rare: polymorphic erythema.

. Very rare: Stevens-Johnson syndrome, Lyell syndrome, bullous or exfoliative dermatitis and generalized acute exanthematous pustulosis (see Warnings and Precautions for Use / Warnings section). These skin manifestations may be more frequent and / or more intense in patients with infectious mononucleosis or developing lymphoid leukemia.

- Renal and urinary tract disorders:

Cases of acute interstitial nephritis, crystalluria have been reported.

Overdose

- The manifestations of overdose can be neuropsychical, renal (crystalluria - see warnings and precautions for use / Precautions for use section) and gastrointestinal.

- The treatment is symptomatic, particularly by controlling the hydro-electrolyte balance. Amoxicillin and clavulanic acid can be removed by hemodialysis.
